Notification of individuals

This section includes notification by individuals or professionals who are not part of an organization (such as a private pilot), and professionals of organizations that do not have their own event reporting system.

To report an event follow the steps below

  1. If this is your first time reporting an event, read the sections of the Event Reporting System carefully to learn about the most efficient way to report an event and the protective and regulatory regime that applies to the process.
     
  2. Complete the appropriate form.

In which cases do I have to report an event?

Remember that there is a list of persons required to report and a list of events required to be reported.

In the event of an accident or serious incident, both the Civil Aviation Accident Investigation Authority and AESA must be notified. 

If the occurrence is not included in the list of occurrences considered to be of mandatory reporting, but can be considered relevant for aviation safety, AESA may be notified on a voluntary basis. Otherwise, the occurrence need not be reported. However, in case of doubt, AESA recommends reporting the event

Accident definition

Any event occurring in the period between the time when a person enters an aircraft with the intention of flying and the time when all persons who entered the aircraft with the intention of flying have disembarked, and during which:

  1. A person suffers fatal or serious injuries
  2. The aircraft suffers structural damage or breakage that would require major repair. 
  3. The aircraft disappeared.

Definition of serious incident

Any event that is related to the use of an aircraft and in which there are circumstances indicating a high probability of an accident. Some examples: 

  • quasi-collision requiring evasive maneuvering,
  • impact on the ground without loss of control,
  • take-offs interrupted or performed from a closed or unallocated runway,
  • landings or attempted landings on a closed or unallocated runway;
  • severe inability to achieve the expected performance during take-off or initial ascent,
  • fire or smoke in the passenger cabin, cargo compartments or engines,
  • events requiring the flight crew to use emergency oxygen;
  • structural failures of the aircraft or engine breakdowns, 
  • malfunction of one or more systems seriously affecting the operation of the aircraft;
  • incapacitation of the flight crew during the flight,
  • amount of fuel forcing the pilot to declare an emergency situation,
  • Raids on the runway classified as Gravity A by ICAO.
