Notification of professionals and organisations
- Pdf notification forms (mainly addressed to individuals and small or medium-sized organisations)

- Use of ECCAIRS by an organisation (thought for small, medium or large organisations). This European Commission software is distributed to Spanish organisations that request it by the AESA SNS.
Persons obliged to notify shall notify the organisation of which they are a member through the channels and formats set out in the organisation’s mandatory reporting system within 72 hours of knowledge of the occurrence. The organisation will be responsible for notifying EASA within 72 hours after becoming aware of the event. Under Royal Decree 995/2013 implementing the regulation of the State Operational Safety Programme for Civil Aviation, the occurrence reporting system of EASA may be notified in addition.
If you are a professional not obliged to notify or the notification is outside your scope, you can make a voluntary notification within your organisation for evaluation and, if applicable, for referral to EASA.
Regulation (EU) 376/2014 requires organisations to use standardised formats for communication with the authority to facilitate the exchange of information compatible with ECCAIRS and ADREP. To facilitate compliance with this requirement, the European Commission has developed acceptable means of compliance:
Before the entry into application of Regulation 376/2014, AESA had an additional reporting medium in Excel format with ECCAIRS compatibility capabilities. In order to allow the sector to adapt to the new means of compliance, this format was being accepted by EASA on a temporary basis and subject to the organisation’s agreement with EASA. However, the notification in Excel as a means of notification is expected to cease to be valid in May 2020, so that users of this method will have to use another of the compatible means thereafter.
If you wish to implement the notification via E5X or ECCAIRS, the organisation should contact the EASA Success Notification System prior to its development to coordinate shipments using these methods.
In the case of accidents and serious incidents, the Commission for the Investigation of Civil Aviation Accidents and Incidents (CIAIAC) remains the official body responsible for managing and conducting its investigation. However, in these cases, in addition to the communication to the CIAIAC, additional notification to the EASA SNS is mandatory provided that Regulation (EU) 376/2014 applies to it.

The forms published here are originally developed by the European Commission and present slight modifications for adaptation to EASA’s Event Notification System (SNS). Given their characteristics, these forms are designed primarily for individuals and small or medium-sized organisations.
To fill out the form you must open it with a PDF reader application that supports PDF forms and embedded scripts. The form has been tested using the available free PDF reader version ‘Adobe Acrobat Reader DC’ which can be downloaded from www.adobe.com. Other PDF readers may not work. For that reason, we recommend using the PDF reader mentioned above.
These pdf formats are designed to be automatically pre-processed by the systems of the Event Notification System, so, once the changes are filled in and saved, the file itself must be sent not being valid prints, photographs or any other process that does not allow such automated preprocessing.
Once completed, you must save it and attach it in an email to sucesos.aesa@seguridadaerea.es
It may be useful to keep a copy of the form sent for future references.
Since old Word forms published on this page do not meet ECCAIRS and ADREP compatibility requirements and do not allow automatic preprocessing, they are no longer valid for notification by organisations.

In the event that the person required to report is not part of an organisation (such as a private pilot) or the organisation does not have an occurrence reporting system, the reporting must be sent, within 72 hours of knowledge of the event, directly to the EASA Success Reporting System (SNS) through the channels set out below.
In the case of notifications by direct individuals to the EASA SNS, there is no obligation to use a predetermined format, however it is strongly recommended that the form be used for individuals present under the item“occurrence reportingforms”.

The forms received are processed by extracting the information that can identify the notifier and, subsequently, this information is destroyed. This ensures confidentiality and protects the notifier’s identity.
In the case of accidents and serious incidents, the Commission for the Investigation of Civil Aviation Accidents and Incidents (CIAIAC) remains the official body responsible for managing and conducting its investigation. However, in these cases, in addition to the communication to the CIAIAC, additional notification to the EASA SNS is mandatory provided that Regulation (EU) 376/2014 applies to it. Furthermore, the Commission for the Assessment and Analysis of Air Traffic Incidents (CEANITA) continues to receive the notifications of such incidents, as referred to in Appendix Y of the Air Traffic Regulation, and then forward them to the SNS for loading in a database.
