Type, class, instrument ratings - Which form has to be filled in by the examiner for extensions?

There is no predetermined form by EASA.

In the case of pilots using the EASA Resolution dated 18.03.2020, this will be done in accordance with what is determined by the corresponding Operator and in relation to the EASA Resolution dated 5 May, it will be the instructor/examiner who draws up the corresponding certificate.
